What services do you offer?
Our team of providers are skilled in managing various health conditions to all our patients. We offer primary care, telehealth services, women’s and men’s preventative health services, chronic disease management, suboxone based medication assisted treatment, uncomplicated outpatient alcohol use disorder treatment, and gender affirming care.  Our skilled team perform in-office procedures including pap smears, skin biopsies, EKGs and more. In addition to scheduled appointments, we also offer acute care visits and same-day appointments for urgent needs. We also work collaboratively with our community colleagues for all specialty services including complex addiction services, behavioral health services and all other subspecialty services.
What ages do you serve?
Our primary demographic is for adults over the age of 18, however, some of our providers see patients as young as 15 years of age. For patients younger than 15 we may make exceptions on a case-by-case basis, but our clinic is not adequately equipped to see younger patients routinely.
Which insurances do you accept?
As of January 2025, we have active insurance contracts with Aetna (Commercial & Medicare), Cigna (Commercial & Medicare), First Choice Health, GEHA (Commercial & Medicare), Medicare (Part B), Meritian, Moda Commercial (Connexus, Synergy, Affinity) & Moda Medicare, PacificSource Community Solutions*, PacificSource Commercial (Navigator) & PacificSource Medicare, Providence Commercial (Signature) & Providence Medicare, Regence BlueCross BlueShield (Commercial & Medicare), United Healthcare Commercial (Charter, Choice, Core, Navigate, Nexus ACO, Select, Doctors Plan) & United Medicare.
*To ensure sustainability for our organization we screen all patients with Medicaid and OHP. Referrals are encouraged, otherwise please contact our office to check for availability.

Each insurance company has several different plans, classifications, or tiers that Pine Springs Health may or may not qualify for (including insurances that are listed above). Pine Springs Health verifies that your insurance is active on the date of service. We do NOT verify if Pine Springs Health is in network for your specific plan. You, the patient, are responsible for knowing your plan and the full extent of your benefits/coverage. You can do this by contacting the phone number on the back of your insurance card or by using your insurance's online portal and searching Pine Springs Health or one of our providers.
What is your philosophy regarding chronic pain management?
We strongly believe that pain does not equal suffering. While pain may not be in our control, suffering, because of pain, is preventable.  We avoid prescribing opioids for chronic pain management. Instead, we offer alternative, evidence based, safer treatments to manage pain.  We do provide compassionate and evidence-based treatment for those who are interested in transitioning away from chronic opioids.
What is your Cancellation Policy?
If you have to cancel or reschedule an appointment we ask that you do so at least 24 hours in advance. If an appointment is canceled less than 24 hours in advance due to emergencies or unforeseen circumstances, please inform us and the management team will determine whether the fee will be charged. Otherwise it will be a No Show.
Our policy is to charge a $50 fee for all No Shows. These charges will be directly billed to the patient. If a patient has three or more no shows within a 24-month period, they may be discharged from the clinic.
If it is your first appointment with Pine Springs Health (New Patient/Establish Care Appointment) then we ask that you cancel or reschedule at least 7 days in advance. Otherwise, this will be a Now Show as well, regardless of reschedules or care transfers. A second New Patient No Show will result in another $50 fee and the patient will be dismissed from our practice.
If a patient arrives more than 10 minutes late for their scheduled appointment, the care team may request that the appointment be rescheduled. This will count as a No Show & a fee will be applied.
How do I become a patient at your clinic?
Please call us at 541-330-9110 to register as a patient at our clinic and get scheduled for a new patient appointment with us!
